The fourth manga volume of Devils' Line was originally published in Japan by Kodansha, Ltd. on January 23rd, 2015. It was published in English by Vertical Comics on December 6th, 2016. It consists of Lines 18 through 22 and Line 1' (side story).

Tsukasa insists on seeing that Oryo is cared for at a hospital, defying Anzai's wish that she stay hidden at their safe house.
